Return of property
By your last day, return:- Practice-issued laptop, phone, or other equipment, with chargers and accessories
- Keys, fobs, and access cards
- Any paper with client or practice information
- Anything else on your equipment form
Client transition
Your clients are the practice’s clients, and they’re also people in the middle of treatment. The transition is handled as a clinical process, together with Helen, starting the day you give notice. The process:- Plan with Helen. Within a few days of giving notice, review your caseload with Helen. For each client: stay with the practice (with whom), continue with you elsewhere (if that’s an option and appropriate), or refer out.
- Tell each client in session, with enough time for a real ending. At least two to three sessions where possible. Tell them their options, plainly, and let them choose.
- Document each conversation and the client’s decision.
- Complete a transfer summary for any client staying with the practice or being referred, so the next clinician has what they need.
- Close the loop. Every client has a documented plan before your last day. No client learns you’ve left from the front office.
- Pressure or recruit clients to follow you
- Contact clients about your departure through any channel except the practice’s
- Take, copy, or photograph any part of a client’s record
- Continue treating a practice client after your last day without their record being properly transferred through the release process
